This form allows users to change their password or the email associated with their Health eLinks® account. This service is available to:

  1. registered users of Health eLinks® who do not already have access to Plan Member e-Services
  2. registered users of Health eLinks® who answered personal security questions during the initial registration process
  3. members who have correctly entered all of the information below
