Plan Member Health eLinks® Change Password or Email
This form allows users to change their password or the email associated with their Health eLinks® account. This service is available to:
- registered users of Health eLinks® who do not already have access to Plan Member e-Services
- registered users of Health eLinks® who answered personal security questions during the initial registration process
- members who have correctly entered all of the information below
